You will take notice that a Petition for the Adoption of a child born to Melissa Ann White, natural mother, set to be heard on the 23rd day of December, 2014, at 11:00 o’clock a.m. alleging that the identity of the natural father of the child is unknown and had not been disclosed to the Court, and whose relationship of said unknown father or undisclosed natural father to the aforesaid child is that of putative father. Minor child’s birth date is July 10, 2006.
